Can Slow the Progression of Cancers A recent article in the renowned journal "Physics Today" explained how low intensity electric fields can slow the progression of certain cancers. The discovery was that electric fields much too small to cause heat can still inhibit the progression of cancer, due to factors concerning the division of cells. Cancers New research in the August Issue of Physics Today explains how low-intensity electric fields can disrupt the division of cancer cells and slow the growth of brain tumors. The team of researchers used alternating electric fields that shift electric particles in cells back and forth hundreds of thousands of times per second. These low-intensity alternating electric fields were once thought to only heat cells but after several years of experiments, the researchers have shown the fields to disrupt cell division in tumor cells placed on a glass dish. After studying the effect of electricity in vitro and in labratory animals, the researchers began a small human clinical trial. The technique was applied to ten patients with glioblastoma multiforme, a type of brain cancer with a low survival rate. The researchers fitted the patients with electrodes that applied 200 kHz electric fields to the scalp at regular intervals for up to 18 hours a day. The cancer was observed to be progressing to advanced stages at a much slower rate (a median of 26 weeks) and in some cases even regressed. Typical time for tumor progression is 10 weeks and 3 of the 10 patients were still alive two years after the electrode therapy started. Electric field therapy is found to have two effects on tumor cells. First, it slows down cell division by disrupting the formation and function of a key cell structure known as the mitotic spindle. Second, the electric field can sometimes cause the daughter cells to self destruct by forcing structures and molecules away from its infrastructure before the cell splits away from its partner . Electrode therapy poses little danger to normal brain tissue, because healthy brain cells do not divide. Researchers are now investigating the possibilities of combining electric fields with low-dose chemotherapy. After the tiny clinical trial mentioned in the above article, a multi center study is now being conducted. The results so far are promising. "So this treatment focuses on the property of the cancer cells, namely that they are rapidly dividing." Principal investigator of the arm of the study at the University of Illinois-Chicago Medical Center, one of a dozen U.S. hospitals conducting the trial, Engelhard can't disclose results while the study is ongoing. But, he says, "I've been very pleased with the results that I've seen so far." Engelhard, a brain surgeon, isn't just an MD. He also has his PhD in tumor biology. "I was skeptical when I first heard about this treatment, I must say that," he says. "But I think you have to keep an open mind about new things, and so I agreed to be one of the investigators in this study. "We've been very happy to find that there is minimal toxicity-- of course we all know about the possible bad side effects of chemotherapy and radiation therapy," he says. "And so far this new type of treatment has been found to be very safe. And what the study is doing is looking to see whether or not it's effective." "There's no cutting, there's no drugs involved at
all, it's very simple, almost too simple," Engelhard
continues. He was impressed by the results of a small safety study, now published in the journal Proceedings of the National Academy of Sciences. "Not only was it found to be safe in the pilot study, it was found that it did improve survival," says Engelhard, who was not involved in any of the device's early studies. In the pilot study in Europe, researchers led by Yoram Palti of the Technion-Israel Institute of Technology tested their prototype device in ten patients with recurrent GBM. While the purpose of the trial was to look for side effects, the authors compared the subjects' time to progression (how long until their cancer progressed), PFS6 (percent that survive six months with no progression) and overall survival with compiled statistics from clinical trials of the best available treatments for recurrent GBM. The treatment exceeded the historical outcomes in all of these measures, more than doubling the expected survival time on average. That led the researchers to ask the US Food and Drug Administration to approve a Phase III efficacy trial without first conducting a large Phase II safety trial. Palti, who invented the device, has formed a company called NovoCure that's funding the research. The Phase III trial, which is still recruiting patients, is a randomized controlled study-- to tell if the device is truly effective, only half of the patients are randomly selected to use the new device. Patients who aren't randomly assigned to the device get the most advanced available treatments. Engelhard says new treatments for GBM are badly needed because the disease is so insidious and, in fact, isn't really a tumor at all. "It might show up on the MRI scan as a circle or a ball, but really, by the time it's seen on the MRI scan, individual cells have gone deep into the brain. So glioblastoma multiforme is really cancer of the brain, and it's very, very difficult to treat," he says. "Once the tumor does not respond to radiation therapy and chemotherapy-- because surgery can't remove it all-- then one does have a very short time left on average." While Englehard acknowledges that the device is "cumbersome," he regards it as a prototype that could be refined if it's proved to work. He also says it's unknown whether it has any long-lasting effects. Quatrano doesn't care if he needs to wear the device for the rest of his life. continues. Cancer CellsCancer cells divide and multiply rapidly in the brain. These cancer cells carry special types of electrically charged elements that play a role during the cell division process. Other healthy cells in the brain multiply at a much slower rate, if at all, and thus rarely include the same electric properties as the dividing cancer cells. Tumor Treating Fields (TTFields)The NovoTTF-100A device used in this trial delivers very low intensity, alternating electric fields to the tumor site through the scalp. These fields are known as Tumor Treating Fields or TTFields. Due to the unique shape of cancer cells when they are multiplying, TTFields cause the building blocks of these cells to pile up in such a way that the cells physically break apart. In addition, cancer cells also contain miniature building blocks that move essential parts of the cells from place to place during division. TTFields cause these building blocks to fall apart since they have a special type of electric charge. As a result of these two effects, preliminary study data indicate that cancer tumor growth is slowed and may even reverse after continuous exposure to TTFields.
